The following are a list of some forms to be used for data collection in the event of a disaster. These forms are currently being used by the California Earthquake Clearinghouse. The Earthquake Engineering Research Institute is updating these forms with the help of the members of the Learning from Earthquakes (LFE) and Information Technology Committees.
